What is a competition "Entry Form" and how do I use it? PDF Print E-mail
This is a 3x5-inch piece of paper used by the Competition Committee to keep track of your photo and award it a score. Entry Forms are color-coded by competition category and distinctively marked by photo medium (color print, monochrome print, or slide). For each photo you are submitting, fill in the appropriate Entry Form and place it with your photo on the collection table. You can pick up Entry Forms at a Forum meeting or print your own off this website. (Note: you do not need an entry form for a digital slide.) See the "Competition" page for details.
